As the starter class, the Intern is designed to get you through your first few games without immediately burning out on Sanity. The +10 passive means you can afford a couple extra mistakes or take on one more patient before needing a break. Prioritize learning the hospital layout, check-in flow, and which upgrades to save for.

Upgrade the Intern to Level 3 only if you are committed to grinding Animal Coins without spending them on a new class. The Level 3 +20 Sanity buffer is decent but still outclassed by the Doctor's on-heal Sanity recovery at 900 coins, which is self-sustaining rather than a one-time buffer. Use the Intern until you can afford at least the Nurse (10 coins) or ideally the Doctor, then switch immediately.
