Patient has eyes sitting at different heights and sharp teeth through a menacing grin. Eurogamer also describes a 'poorly drawn' variant with hand-drawn, misproportioned eyes and a large mouth with crooked teeth.

Patient has different eyes in their photo than they do in person. Can be subtle - different eyelids, pupils, etc.

Wait for the photo to develop on the desk. If it looks odd, close the shutters before picking it up.

Patient has different ears in their photo. More obvious than some eye differences but still requires careful checking.

Close the shutters if the photo doesn't match the patient.

Photo shows a completely different character model - different colors, markings, or even a different animal entirely.

Photo shows bloodshot eyes, grotesque smile, or blurred features. Looking at these pictures costs you sanity. Covers a wide variety of anomalies.

Observe photos on the desk before picking them up to avoid sanity damage. Close shutters if the picture doesn't match 1:1.

Character has a big black rectangle covering their eyes with another pair of eyes drawn on top (IGN) / A JPEG of white eyes on a black background overlaid on the patient, staring into the camera (Eurogamer).

Patient has an elongated or otherwise unnatural body on CCTV. Limbs stretched and often fully detached. Can be very long, tall, or wide with limbs at strange angles.

Same hollow face expression visible in person can also appear on CCTV. Hard to spot especially if the anomaly isn't looking at the camera.

Use the waiting room camera and check the patient's eyes closely.

Staring At Camera / Camera Watcher

Anomaly stares straight at the camera when viewed through CCTV. Normal patients should be staring straight ahead at the office.

Void Bodies / Shadow Patient

Patient's body is completely black on CCTV with no discernable features (IGN) / Patient turns into a shadow when viewed through CCTV (Eurogamer).

Close the shutter.

Skinwalker (CCTV)

Wide open mouth with sharp teeth and long tongue visible on CCTV. Not visible in person or on photographs.

Mismatched Ears or Eyes (CCTV)

Patient's ears or eyes don't match the animal standing in front of you when viewed on CCTV. The hardest anomaly to spot since cameras are far away.

First note the patient's ear and eye shape in person, then check the CCTV. Close shutter if mismatched.

Twitching (CCTV)

Patient may twitch on CCTV but not in person. Watch for at least 4-5 seconds to avoid missing quick twitches.

Close the shutter if twitching observed on camera.
Highmonsters

Bed Monster / Monster Under The Bed

Large black hands poke out from under the mattress with a red line on the floor. A large red box on the floor that must be avoided. If you touch the box, the monster will catch you and drag you under, dealing sanity damage.

Avoid the red line, hop over the bed. Treat the patient and the monster will leave. Loves Maple Syrup - give it some to make it leave safely.
Lowmonsters

Head Banger / Head Knocker

An unsettling anomaly with a hollow face and sunken eyes that walks into the office, approaches the window, and starts banging its head against the glass for a couple of minutes before cracking the glass and leaving.

Close the shutter while it's rearing back to headbutt. Can't use shutters normally until the monster disappears. Inconvenient but not lethal.
Very Highmonsters

Skinwalker (Hospital Roaming)

Monsters that emerge from anomalies that were let into the hospital. They can transform at any time after admittance. They let out a roar when emerging. Look out for heavy breathing and an exact copy of a patient you've already seen.

Use a gun or taser to shoot the monster. You can also hold E to forcefully dislodge them from their victim, or spam E to punch them if they attack you. Beat the timer before they eat you or a patient.

The Stalker

A tall, black shadowy bunny entity that hides around corners. Deals sanity damage if you look straight at it. Peek around corners at you.

Avoid eye contact if possible. If you do look, you lose 10 Sanity. Not a massive deal but adds up.
Mediummonsters

Tendril

Large purple tentacles that emerge from patients in the surgery room. Exceedingly rare and only appears in Room 8.

Complete the patient's surgery before the timer runs out. When the mini-game is complete, the tendrils vanish. If you don't make it in time, the patient dies.

Camera Anomaly / Demon Cat Jumpscare

IGN: Appears in broken CCTV cameras, slowly zooms in on the monster. Eurogamer: A demonic cat entity with glowing red eyes, visible only through cameras, standing at the end of hallways. The camera zooms in cueing a jumpscare.

IGN: Press the Exit button along the bottom of the screen in time, then fix the camera. Eurogamer: Can't avoid outside of not using cams. Jumpscare costs 20 Sanity.

Don't Look Up / Flesh Ceiling

A hideous blob of flesh with eyeballs that weeps on the ceiling. You'll hear groaning and see dripping saliva. Game flashes 'don't look up' on screen.

Don't look up! Looking causes steady sanity damage. To remove: hold Eye Drops, open the door facing inside, jump on the door, slowly look toward the monster until 'Accept Treatment' text appears.
Highmonsters

Hiders / Translucent Man

IGN: Camouflaged monsters that hide against surfaces, breathe very loudly, appear in groups of 3. Eurogamer: A translucent man that blends into the tiles of a patient's room with a pair of eyes and moving body on the wall.

IGN: Listen for raspy breaths, scan the room, see their eyes and smile. Avoid or use gun/taser. Eurogamer: Run away to the lobby and the translucent man disappears in a puff of smoke.

Death Ritual

Patient floats above their bed, surrounded by candles and red light. Ominous sounds emerge from the room and all nearby tech glitches out.

Beat the timer and get to their room, then remove all the candles. Removing each candle depletes your own Sanity, but failure means the patient dies.
Lowmonsters

Slime

A patient leaking bright green slime onto the floor as they walk by.

Interact with each glob of slime to clean it up.
Mediummonsters

Fire

An empty patient room catches on fire, engulfing the area.

Grab the fire extinguisher (right of doors to the shop) and extinguish the fire.
