Patient has an elongated or otherwise unnatural body on CCTV. Limbs stretched and often fully detached. Can be very long, tall, or wide with limbs at strange angles.
Same hollow face expression visible in person can also appear on CCTV. Hard to spot especially if the anomaly isn't looking at the camera.
Anomaly stares straight at the camera when viewed through CCTV. Normal patients should be staring straight ahead at the office.
Patient's body is completely black on CCTV with no discernable features (IGN) / Patient turns into a shadow when viewed through CCTV (Eurogamer).
Wide open mouth with sharp teeth and long tongue visible on CCTV. Not visible in person or on photographs.
Patient's ears or eyes don't match the animal standing in front of you when viewed on CCTV. The hardest anomaly to spot since cameras are far away.
Patient may twitch on CCTV but not in person. Watch for at least 4-5 seconds to avoid missing quick twitches.
Large black hands poke out from under the mattress with a red line on the floor. A large red box on the floor that must be avoided. If you touch the box, the monster will catch you and drag you under, dealing sanity damage.
An unsettling anomaly with a hollow face and sunken eyes that walks into the office, approaches the window, and starts banging its head against the glass for a couple of minutes before cracking the glass and leaving.
Monsters that emerge from anomalies that were let into the hospital. They can transform at any time after admittance. They let out a roar when emerging. Look out for heavy breathing and an exact copy of a patient you've already seen.
A tall, black shadowy bunny entity that hides around corners. Deals sanity damage if you look straight at it. Peek around corners at you.
Large purple tentacles that emerge from patients in the surgery room. Exceedingly rare and only appears in Room 8.
IGN: Appears in broken CCTV cameras, slowly zooms in on the monster. Eurogamer: A demonic cat entity with glowing red eyes, visible only through cameras, standing at the end of hallways. The camera zooms in cueing a jumpscare.
A hideous blob of flesh with eyeballs that weeps on the ceiling. You'll hear groaning and see dripping saliva. Game flashes 'don't look up' on screen.
IGN: Camouflaged monsters that hide against surfaces, breathe very loudly, appear in groups of 3. Eurogamer: A translucent man that blends into the tiles of a patient's room with a pair of eyes and moving body on the wall.
