The Doctor is widely regarded as the best free-to-start class in Animal Hospital. At 900 Animal Coins, it sits comfortably in the mid-tier price range, but its self-sustaining Sanity mechanic punches far above its cost bracket. Every time you heal a patient, you recover 1 Sanity (plus the bonus Sanity from upgrades). This creates a natural feedback loop: treat patients → gain Sanity → stay calm enough to treat more patients. Unlike classes that rely on consumables or external sources for Sanity management, the Doctor generates it passively through the core gameplay loop of doing your job.

The Sanity self-loop is especially valuable in longer shifts and higher-difficulty scenarios where Sanity drain ramps up. As anomalies accumulate and environmental hazards chip away at your mental state, other classes are forced to burn through Coffee, Chocolate, or other consumables just to stay functional. The Doctor simply keeps healing — and each heal is a mini Sanity potion. At Level 3, the +20 bonus Sanity on top of per-heal recovery means you start every shift with a substantial buffer, making it very hard for Sanity to become a problem even in chaotic rounds.

For new and intermediate players, the Doctor is the ideal stepping stone. It teaches solid healing habits (prioritize patients, stay active, keep moving) while naturally managing the resource that causes the most trouble for beginners: Sanity. Once you are comfortable with the Doctor's playstyle and have saved up enough coins, the Surgeon (2500 coins) offers a direct upgrade with speed boosts layered on top of the same Sanity-on-heal foundation.

Comparison to Adjacent Tier Classes

The gap between the Paramedic and the Doctor is one of the most significant tier jumps in the game. The Paramedic offers speed, which helps with response time but does nothing for Sanity or healing output. The Doctor, for 3.6 times the cost, introduces a self-sustaining Sanity engine that fundamentally changes how you play — you no longer need to fear Sanity drain as long as you keep healing. Compared to the Nurse at the bottom end, the Doctor is in a completely different league; the Nurse's single extra inventory slot is a convenience, not a survival tool. For players deciding where to spend their first significant coin investment, the Doctor at 900 coins is almost always the correct answer before thinking about the Surgeon at 2500 coins.
