S Tier — Best Classes
Surgeon (2,500 Animal Coins)
The best class available for free-to-play players. Every heal grants Sanity recovery AND a speed boost, making you completely self-sustaining through long shifts.

Secret Agent (320-890 Robux)
The overall strongest class if you spend Robux. Starts with a Gun (20+ uses), which is the most powerful anomaly elimination tool in the game.

A Tier — Excellent Choices
Doctor (900 Animal Coins)
The best budget option before Surgeon. Recovers Sanity on every heal, keeping you healthy throughout the shift.
Psychologist (500 Animal Coins)
High risk, high reward. All Sanity effects are doubled — both positive AND negative. At Level 2+, gains a chance to ignore Sanity loss entirely.
B Tier — Solid Options
Secretary (120 Animal Coins)
Recovers Sanity when checking in patients. Great for front desk duty during early-game when patient flow is steady.
Head Nurse (80-190 Robux)
+3 max inventory capacity. Well-rounded utility. IGN ranks it S-tier; TechWiser ranks B-tier due to Robux cost.
Security (1,250 Animal Coins)
Starts with X-Taser (5 uses). Good for anomaly suppression, but the Taser is freely available in the Reception Office.
C Tier — Niche
Paramedic (250 Animal Coins)
Starts with Large Speed Cola. Movement speed helps but doesn't prevent Sanity drain or patient deaths.
Nurse (10 Animal Coins)
+1 inventory capacity for nearly nothing. No Sanity passives.
D Tier — Starter
Intern (Free)
Default class. Only provides a small Sanity boost. Upgrade ASAP.
