Beyond anomalies, the hospital is home to dangerous monsters and environmental hazards that can kill you, drain your Sanity, or destroy equipment. Knowing what each one does and how to respond is essential for survival.

1. Bed Monster

A creature that hides under hospital beds. You'll see a red line or glow beneath the bed indicating its presence.

  • Danger Level: 🔴 High
  • How to handle: Give the patient Maple Syrup. This calms the Bed Monster and makes it retreat.
  • Warning: Ignoring the Bed Monster can lead to it attacking you or the patient.
  • Tip: Always check under beds when entering a room. The red line is your early warning system.

2. Head Banger

A figure that appears at the reception window and repeatedly bangs its head against the glass.

  • Danger Level: 🟢 Low
  • How to handle: Close the shutter while the Head Banger is rearing back for another strike. Timing is important — close it during the backswing, not while it's making contact.
  • Tip: This is one of the easiest hazards to deal with. Don't panic, just time the shutter close.

3. Skinwalker

A dangerous entity that roams the hospital corridors, mimicking the appearance of patients or staff. It is one of the most lethal threats in the game.

  • How to handle:
    • Shoot it with a gun (most reliable).
    • Use a taser to stun it temporarily.
    • Hold E near it to trigger an interaction that may repel it.
  • Warning: Do not engage in melee or try to outrun it. Use your tools.
  • Tip: Keep your gun or taser equipped when moving through corridors, especially late in a shift.

4. The Stalker

A humanoid entity that stares at you from a distance. Making eye contact with The Stalker costs 10 Sanity.

  • How to handle: Avoid looking at it. Turn away, look at the floor, or focus on your tasks while it's present. It will eventually leave.
  • Sanity cost: -10 per stare
  • Tip: If you notice a figure in your peripheral vision that seems to be watching you, DO NOT look directly at it.

5. Tendril

A fleshy tentacle that appears in the surgery room and grows over time. A timer starts when it spawns.

  • How to handle: Complete the surgery before the timer runs out. If you finish the surgery in time, the Tendril disappears. If the timer expires, it attacks.
  • Tip: Prioritize speed in the surgery room when a Tendril is present. Don't waste time on optional steps.

6. Camera Anomaly / Demon Cat

An anomaly that appears on your camera feeds, manifesting as a demonic cat-like figure. Triggering it results in a jumpscare.

  • Danger Level: 🟡 Medium (Sanity threat)
  • How to handle: Press the Exit button on the camera interface to leave the camera view immediately when you spot it.
  • Sanity cost: -20 if the jumpscare triggers
  • Tip: If something on the camera looks like a cat, Exit immediately. Don't wait to inspect it.

7. Don't Look Up / Flesh Ceiling

The ceiling becomes a fleshy, organic surface that damages you if you look up at it.

  • How to handle:
    • Use Eye Drops to temporarily protect yourself.
    • Use the door trick — stand in a doorway where the ceiling is normal and look up briefly to check corridors without full exposure.
  • Tip: Keep Eye Drops in your inventory for emergencies. This hazard punishes curiosity.

8. Hiders / Translucent Man

Semi-transparent entities that hide in rooms and can only be detected by listening for their breathing sounds.

  • How to handle: Listen carefully for breathing when entering a room. If you hear it, use your gun to shoot the Hider before it can attack.
  • Tip: Turn up your game audio or wear headphones — the breathing sound is subtle but distinct. It's your only warning.

9. Death Ritual

A patient begins floating in the air, surrounded by candles in a ritual formation. A timer counts down.

  • Danger Level: 🔴 High
  • How to handle: Remove all the candles before the timer expires. Interact with each candle to extinguish it. If the timer runs out, the ritual completes and the patient is lost (or worse).
  • Tip: Move quickly between candles. The timer is tight, especially with multiple candles to extinguish.

10. Slime

A slippery green substance that appears on the floor.

  • Danger Level: 🟢 Low
  • How to handle: Interact with the slime to clean it up. It's a minor hazard but can slow you down or cause slipping.
  • Tip: Clean slime promptly to keep pathways clear, especially in emergency situations.

11. Fire

Fires can break out randomly in the hospital.

  • How to handle: Use a fire extinguisher to put out the fire. Fires can spread if left unchecked and may destroy equipment or block paths.
  • Tip: Know where fire extinguishers are located before you need them. Keeping one in your inventory is ideal.
